This is a needs assessment phone call with Emily to understand your behavioral health needs and support connection to providers; Emily will call you at your contact. Your care is billed to your insurance like a regular medical visit under your primary care office, not from Benchmark directly. Depending on your plan, your usual cost sharing may apply: a copay, coinsurance, or a deductible, the same as other visits with your PCP. Many people pay little or nothing, but every plan is different, please feel free to ask Emily about the code used before the call if you have any questions.

This is a 20 minute follow up call to check in and see how things are going. Nothing to prepare.
